Welcome to Asthma and Allergy Associates, Sterling/Troy, P.C.. We look forward to helping you with your allergy and/or asthma symptoms. Our goal is to provide the highest quality care for all of our patients in a timely and respectful manner. Please have your paperwork fully completed when you arrive for your appointment or we may need to reschedule your appointment for a later date. We ask that you arrive 10-15 minutes early for your appointment so that we can get you registered and in a room as close to your appointment time as possible. Patients who are 15 minutes late or more may need to reschedule their appointments depending on our schedule that day. Please call 48 hours ahead to cancel your appointment or a $25.00 charge will be assessed. New patient appointment slots do take an hour out of the doctor’s schedule and last minute cancellations and no shows affect our schedule greatly.
It is in your best interest to call your insurance company before your appointment and verify your benefits so that there are no unexpected charges to you. We do our best to verify your benefits in the office but are not always able to guarantee this. If your insurance company requires referrals for specialists, it is your responsibility to get that referral for your initial appointment and also all follow-up appointments
The following codes will help you to verify coverage with your insurance company. Please have them available when you call your insurance company to verify coverage:
If any of the above charges are non-covered services, please let the physician know at the time of the service whether or not you would like them performed. All uncovered services, co-pays, co-insurance and deductible charges are due at the time of service. If you must set up a payment plan that is something you can discuss with the receptionist at the time of service but 50% of your charges must be paid upfront.

IF YOU ARE ON ANY MEDICATIONS, VITAMINS OR SUPPLEMENTS PLEASE LIST THEM ON THE MEDICATION LIST IN THE DOWNLOAD OR BRING A MEDICATION LIST WITH YOU. IF YOU ARE TAKING ANY INHALERS PLEASE BRING THEM TO YOUR APPOINTMENT WITH YOU.
Remember to bring your insurance card and Identification along with your paperwork.
ALL PAPERWORK MUST BE COMPLETELY FILLED OUT WHEN YOU ARRIVE FOR YOUR APPOINTMENT OR YOUR APPOINTMENT MAY BE RESCHEDULED.
No over-the-counter or prescription antihistamines 3 days before your appointment
Testing cannot be performed if you have antihistamines in your system, it will block our allergy testing.
If you are experiencing hives, you may continue the antihistamines and have testing done at a later date.
We will not test anyone who is having hives, rash, wheezing or is currently having an allergic reaction at the time of the appointment.
Please continue ALL inhalers, asthma medications, and non-allergy related medications prescribed by your doctor.
